1. Online Orders
- $99.95: Fill out all the required fields on the form
below. Payment is by credit card only. You must include a valid credit
card number, email and mailing address. Upon credit card approval the information manual will be sent to you by first class mail.
2. Mail Orders - $79.95: If you would like to pay
by a method other than credit card, please print
out the mail order form, fill it out completely, and mail it to the
address indicated on the form. Upon receipt of your order, the
information manual will be sent to you by first class mail.
